Consent Forms
When you are admitted, you will be asked to sign a consent
form giving us permission to perform routine care. We are required
to make sure you fully understand all the risks and benefits
associated with any procedures you may have to undergo.
If the patient is a minor or too ill to sign we will ask the
appropriate next of kin to sign. Of course, in an extreme
emergency, in order to protect the patient and to provide the best
possible care, we may have to proceed without permission.
We want you to feel confident about the care you receive, so
please feel free to ask questions. We will be happy to take the
time to explain.
